Carotid Artery Stenosis and Stroke
A series of randomized, controlled studies have clarified the benefits of carotid endarterectomy for stroke prevention. Carotid endarterectomy is a procedure where plaque is surgically removed from the carotid artery to prevent the plaque from breaking off and traveling to the brain. Rockland Thoracic and Vascular Associates P.C. boasts one of the lowest complication rates for carotid surgery in the state of New York. A majority of our patients are discharged on the first post-operative day. Duplex ultrasound imaging for carotid disease is done in the setting of our ICAVL certified in-house vascular laboratory making the diagnostic work-up and follow-up evaluations convenient.
We also offer carotid angioplasty and stenting for the minimally invasive treatment of carotid artery narrowing when indicated. Stenting of the carotid artery is a new procedure where a stent is placed into the carotid artery eliminating the need for a carotid surgery. The benefit of this new technique is being studied to determine its effectiveness. Rockland Thoracic and Vascular is proud to be a site for the SAPPHIRE Trial (Stenting and Angioplasty with Protection in Patients at High Risk for Endarterectomy). Being a part of this trial allows appropriate patients to undergo this new technique when indicated.
Lower Extremity Circulation Problems
The last two decades have witnessed enormous advances in the treatment of circulatory disorders of the lower leg. Years ago, patients with poor circulation due to diabetes or other atherosclerotic disorders often faced major amputation. Our aggressive approach to limb threatening circulatory disorders has prevented the need for amputation in most cases. We offer a broad spectrum of treatment options to the patient with lower limb vascular disease.
In most cases, where the leg is not in jeopardy, the treatment is conservative and may only involve life-style alterations, exercise programs and/or medication. In cases where a more aggressive is indicated, a multi-modality approach may be used including minimally invasive interventional therapy (i.e. balloon angioplasty, atherectomy, stent placement, thrombolysis).
In some cases, a minimal invasive approach is not possible and surgery may be indicated. This may include complex bypass procedures when necessary to prevent limb loss. Rest assured, the surgeons at Rockland Thoracic and Vascular are very well trained and experienced at leg bypass surgery. Our patients are closely followed in our office and our accredited vascular laboratory to insure a durable, long-lasting result.
We have a large experience in the treatment of aneurysm disease. This is reflected in our excellent results of elective and emergency cases. Our aneurysm patients benefit from a multi-disciplinary approach to their care. The combination of cardiac anesthesiologists, medical consultants and well-trained surgical nursing staff provide for a successful transition from the preoperative to postoperative phase.
For aortic aneurysm disease, we are able to offer our patients a minimally invasive option to conventional surgery. We are very proud of our aortic aneurysm endovascular repair program, which is the most experienced and boasts one of the largest volumes in the Hudson Valley. In fact, Rockland Thoracic and Vascular Associates was one of the first community practices to offer this technique in the country. Endovascular aneurysm repair entails the placement of a graft to repair the aneurysm through the femoral arteries in the groin. In most cases this is done percutaneous (via a needle stick in the groin). Most patients go home the next day and return to normal activity in 1 – 2 weeks.
Currently, we have performed hundreds of endovascular aneurysm repairs since its inception. We are one of the busiest aneurysm surgery centers in the regional community and perform this procedure weekly. This gives us access to the newest devices available to treat this complex disease.
We also have significant experience in treating complex thoracic aortic disease including aortic dissection and thoracic aneurysms. This can also be treated with a novel endovascular technique.
Dialysis Access Creation and Management
For the hemodialysis patient, we provide dialysis access services. The goal of treatment is to maximize the use of a well planned and well-constructed access in order to minimize the impact that dialysis can have on a patient's life. To meet this goal, procedures are timely scheduled in order to decrease the need for temporary dialysis catheter use. We have decades of experience in dialysis access giving our patient an edge in obtaining a high quality long lasting result.
In addition to creation of dialysis access we are also able to offer state-of-the-art dialysis access maintenance. This allows our patients to continue uninterrupted dialysis. Our patients are followed routinely in our office in order to detect and treat access related problems. In the majority of cases, minimally invasive techniques including thrombolysis, balloon angioplasty and stenting can be used to keep an access functioning.
We are proud to offer access management treatment in our well-equipped outpatient access center. All procedures can be done on an ambulatory basis in our comfortable unit avoiding the need for hospital admission.
Venous Disease and Varicose Veins
In conjunction with our Vein Center we offer a wide range of therapy for our patients with varicose veins and spider veins. All procedures are done in the comfortable environment of the beautiful offices at The Rockland Vein Center
. We also diagnose and treat the sequelae of deep venous thrombosis and phlebitis.
